Our Health Projects aim to reduce health inequalities and improve access to health services for people with a learning disability and autistic people. We do this by providing practical support, accessible information and opportunities for people to build confidence in managing their own health and wellbeing.

We also work in partnership with the Primary Care Networks in North Shields and North West North Tyneside to offer personalised support to patients who have a learning disability.

Our team works with GP surgeries to identify patients who would benefit from additional help to manage their health and wellbeing, particularly those who feel anxious or have concerns about attending health‑related appointments.

Our Care Co‑ordinators support patients to attend annual health checks, ensure accessible information is provided, and help people link with additional health and care services when needed.
